- With your application, attach a copy of your
2) Proof of Address (Passport/Ration Card/Drivers License/Voters ID), and
3) Bank Passbook or Account Statement with details of the account and NEFT code.
- Please note that applications will not be accepted without correct documentation. There are no exceptions. Also note that there is not a membership fee to become a distributor with Mona Vie.
- Hand deliver or mail the completed application to the MonaVie India Chennai Office (see address below).
- We highly recommend that you purchase the Distributor Kit when turning in the application to get started on the right track. It contains a wealth of information on CD's, DVD's and brochures. The MonaVie India Distributor Kit is Rs.800.00 per application and is to be paid in the form of Demand Draft only (individual DDs) in favor of "MonaVie India Enterprises Pvt. Ltd."
- Delivery of the Distributor kits will begin the third week of October. Please make sure to bring proper proof of address for shipping (they will accept a telephone billing statement).
